Why Repeat Incidents Keep Happening and What You Can Do About It

Most EHS professionals have first-hand experience of this. Incidents that could have been prevented but occur because the lessons from previous incidents are not effective. In this session, we dive into this frustrating issue. Look at the ways in which they occur, including the not so obvious issues.
You’ll hear some success stories and strategies for how to improve the effectiveness of the incident management lifecycle in preventing repeat incidents, including how industry is working together to share lessons learned more effectively.

David Rocha is the Senior Product Manager for Health & Safety, where he is responsible for products such as Incident Management, Observations, Job Safety Analysis, Industrial Hygiene, Occupational Health and Ergonomics. David is an EH&S professional with over 10 years in the oil & gas industry where he worked a variety of operational and corporate HSE roles, including development and management of EH&S management systems. David’s passion lies in using his experience to help ease the burden of fellow EH&S professionals in making industry safer.
Stuart King, Good Practice Manager – Human Performance and Power Systems, Energy Institute
Following an MA in Social Anthropology, Stuart King became involved in human and organisational factors through coordinating the EI Hearts and Minds safety culture toolkit activities at the Energy Institute. He is the secretary of the EI Human and Organisational Factors Committee (HOFCOM), the Power Systems Committee (PSC) (along with its working groups – the Energy Storage Working Group and Solar Projects Working Group), and the Learning From Incidents Steering Committee.
